This says a lot: One of best practices now adopted within the Great Lakes Fleet, according to the NTSB report: a reduction of standing verbal orders in favor of written ones.

That's quite an interesting read. They seem like there must have been some preoccupation with the tow. It might be like the Navy / Container Ship collision where it won't just fall on one shoulder. Of course, the Gros Cap Reef Lighthouse is there, but I wonder if a buoy marks the start of the reef to the West.
