I'm not passing judgement on the cause of this incident. We don't know at this point.
However, the suggestion for a cardinal buoy seems unnecessary. A quick glance at the chart shows the #4 red buoy and the shoal right next to it. It should be obvious which side of the buoy to pass without even seeing its color or number. The waters in that area are populated by many obstructions, hence various crooked channels are marked with numerous buoys to keep mariners out of trouble.
